Changing table into long table to span multiple pages

I used the tabular/table environment in order to create a table. The problem is the table is longer than 1 page and from what I understand I need to use the long table environment to separate the table into two. I've tried adapting my script into long table but keep getting errors. can someone tell me where I'm going wrong and possible correct it?
